The Town of Southborough (through Youth and Family Services) is conducting a Community Health Needs Assessment to better understand the needs, challenges, and experiences of our residents.
The survey is anonymous, takes less than 10 minutes, and asks about:
- Physical and mental health
- Stress and substance use
- Access to healthcare
- Basic needs like housing, food, and transportation
Your input will help guide how the Town plans and improves programs, services, and health initiatives in the years ahead.
The survey is:
- Open to all Southborough residents
- Confidential — responses are anonymous and analyzed in aggregate
- Available in English, Spanish, Portuguese, Chinese, Russian, and Arabic
